How To Apply

If you are a resident of Hawaii and are interested in applying for an Assistance Dog, the staff at HCI would be happy to help. You must be able to demonstrate a need for an Assistance Dog and to describe the ways in which a dog will increase your independence and assist you. Please call 808-250-5799 for an application package.
